Event sponsorship enquiry form
A sponsorship enquiry is not a booking, it is the two minutes that decide which pack gets sent. This one asks the three things a partnerships team has to guess at otherwise: roughly what the budget is, what the sponsor actually wants, and when they have to decide. It is conversational, one question per screen, because an enquiry is a conversation and nine boxes at once reads as a credit check.

What makes this form work
The budget question explains itself before it asks
Nobody volunteers a number to a stranger unless they know why it is wanted. Saying that the pack is genuinely different at each level, and that guessing wastes a fortnight for both sides, turns the most avoided question on the form into the one people answer honestly.
The deadline is stated, not requested
Print and signage close on 6 March, so a sponsor deciding in April gets a smaller set of options. Putting that beside the timing question means the sponsor is choosing with the same calendar the festival is working to, rather than finding out afterwards.
The contact block asks for a work address and says why
A free mail address is not refused, it is simply slower, and the form says so. That is more effective than a validation rule, and it never blocks the small operator whose business really does run from a personal address.
Questions people ask
Should a sponsorship enquiry form ask for a budget?
Yes, as a band rather than a number, and with a reason attached. A five thousand dollar sponsor and a forty thousand dollar sponsor need completely different conversations, and sending the wrong pack costs both sides a fortnight. The option to say "show me the pack first" keeps the cautious ones in.
Why not ask which tier they want?
Because nobody knows before they have seen what is in each one, and a tier list on an enquiry form makes people self select downwards. Ask what they want to achieve instead. Two sponsors at the same price often want opposite things, and that answer is what lets you build the package.
What happens to the enquiry after it is submitted?
This form collects it. From there an email alert can land in the partnerships inbox the moment somebody submits, and the whole submission can be pushed to a webhook so it lands in your own system. The webhook fires once with no retry, so treat it as a nudge rather than the record.
Is a conversational form right for a business enquiry?
For an enquiry, yes. Nine questions on one page reads as a qualification gauntlet and gets abandoned. One at a time, on a phone, from a link in a prospectus email, is how most of these actually arrive, and a form abandoned at question four still leaves you a company and a contact.

Webinar registration form

A webinar form can be this short because the deliverable is an email, so the only fields that survive are the ones that either send the link or change what the speakers prepare. It is conversational, one question per screen, because that is where webinar links actually get opened.
Tournament registration form
One form, one team, and the tournament office needs all of it before it can draw a pool. This entry takes the division nomination and the evidence behind it, fourteen players in a table, two adults reachable on the weekend, and the child safety checks that decide who is allowed on the bench. It is a classic form, because a squad table and a fee ladder do not survive being broken into single questions.
Gala dinner booking form
A black tie fundraising gala sells most of its room to organisations, on invoice, weeks before anybody knows who is actually coming. This form is built for that reality: it takes the booking and the money trail now, and leaves the ten guest names to a second email in October. It is a classic form, because a booking that ends in an invoice has to be read end to end before it is sent.
